Working day-in and day-out can be stressful at times, but when you are injured on the job it can become frustrating and even more stressful. Injuries on the job can include slips, trips, & falls and can range from back strain, repetitive motion strains, to more serious injuries. Thankfully, some MN Workman’s Compensation insurance** do allow Massage Therapy as an alternative treatment for some claims.
A health intake form and an Insurance Release form will need to be filled out prior to the start of your first session as well. You can choose to fill it out at the office the day of your appointment, or you can fill it out early, and bring it in the day of your first session by downloading the forms below.
Please Note: As of March 1st, 2016, ND Workforce Safety & Insurance (NDWSI) is no longer allowing Licensed Massage Therapists to perform massage therapy services for their injured workers. It is a very unfortunate decision that they have made and it is something I am working on trying to reverse, but in the meantime: Physical Therapists (PTs), Occupational Therapists (OTs), and Chiropractors are the only medical professionals allowed to do this service for ND injured workers.
**MN Workman’s Compensation is through individual insurance companies, each case is different and they may or may not cover Massage Therapy. I will need to contact your case adjuster prior to any sessions to verify if Massage Therapy is covered or not.
